2. Rh Factor

Each blood group is either:

  • Rh-positive (Rh⁺)

  • Rh-negative (Rh⁻)

The Rh factor becomes important only during pregnancy, not in the relationship itself.


Rh Factor Compatibility in Marriage

When Does Rh Incompatibility Occur?

Rh incompatibility can occur when:

  • The woman is Rh-negative

  • The man is Rh-positive

In this situation, the baby may inherit the Rh-positive factor from the father.


Why Is This a Concern?

If an Rh-negative woman carries an Rh-positive baby:

  • Her immune system may recognize the baby’s blood as foreign

  • She may develop antibodies against Rh-positive blood

This usually does not affect the first pregnancy, but it can cause complications in future pregnancies if not managed properly.


Hemolytic Disease of the Newborn (HDN)

If Rh antibodies are already present in the mother’s body, they can:

  • Destroy the baby’s red blood cells

Modern Medical Solution (RhoGAM Injection)

Thanks to modern medicine, Rh incompatibility is completely manageable.

  • Doctors give an injection called Rho(D) immune globulin (RhoGAM)

  • It prevents the mother from forming antibodies

  • Given during pregnancy and after delivery

With proper medical care, Rh incompatibility is NOT a reason to avoid marriage or pregnancy.

ABO Blood Group Compatibility and Pregnancy

The ABO blood group system:

  • Does not cause pregnancy complications

  • Does not cause genetic diseases

  • Does not affect fertility

Examples of safe combinations:

  • O + A

  • B + AB

  • O + O

All ABO blood group combinations are safe for marriage.

Genetic Blood Disorders: A More Important Factor

While blood group compatibility is manageable, genetic blood disorders are more critical to consider before marriage.

Sickle Cell Genotype

Common genotypes include:

  • AA – Normal

  • AS – Carrier (trait)

  • SS – Sickle cell disease

High-Risk Combinations:

  • AS + AS → 25% chance of an SS child

  • AS + SS → High risk of affected children

  • SS + SS → 100% chance of sickle cell disease

  • Unlike Rh incompatibility, sickle cell disease cannot be prevented with injections and may severely affect a child’s quality of life.

    Other Genetic Conditions to Screen

    • Thalassemia

    • G6PD deficiency

    • Hemophilia (if family history exists)

    Premarital medical screening and genetic counseling are strongly recommended.
